Intermediate care centres are intended to be a safe alternative to hospitalization under a comprehensive and transdisciplinary care model that can respond to highly complex end-of-life situations, also in paediatrics. We present a case study of an end-of-life care process in which continuity of care between two different levels of care — a paediatric palliative care unit of an acute hospital and a paediatric intermediate care centre — made it possible to provide quality palliative care to the patient and her family. Based on this, we suggest that decision-making about the place of death should be individualized, respecting the desired place of death for the patient and family, in order to ensure quality care in the best possible environment.
